Our person-centred approach

 

Focus Care Services is a dedicated provider of Community Based Support and Enablement Services for a range of Service User groups; registered under Part II of the Domiciliary Care Agencies Regulations 2002 and complies with the Statutory Regulations inspected by the Care Quality Commission.

 

Since 2003, Focus Care Services has provided a Supported Living service for adults with a learning and or physical disability, sensory, visual impairment, functional mental health, autistic spectrum disorder, substance dependency, dementia and seizures. Our primary aim is to promote Independent Living for people who use our services within a community setting by following “Ordinary Life” principles. This is achieved by involving people who use our services through working in partnership with the service user, their Carer and significant other stakeholders. We create a unique person-centred plan that takes into account individual outcomes. We also recognise that service users wishes and needs change over time and that it is essentially best practice to monitor, evaluate and adopt care plans according to need.

 

We work towards maximising the independence of service users with the aim of reducing the level of support provided over time through agreement with the Service User and Partner Organisation(s). For those reasons, Focus Care Services are known for our flexibility and innovation. The care plan sets out the particular pattern of support and the specific needs and desired outcomes of the service user. Our information is built on the Community Care Assessment and Care Plan provided by the Council and clarifies day to day Support issues. We ensure that the Community Based Support and Enablement Services are provided in compliance with health and safety regulations. We provide a signposting service ensuring that people who use our services have a range of leisure options within the financial resources available. Our services are built around each individual person in order to support them to access opportunities open to all within their communities. We operate on continuous improvement principles to provide purchasers with “best value” services. As individual budget become more widespread; Focus Care Services is developing new systems and processes to enable each person and their families to effectively manage their budgets and direct payments. By adopting a person-centred approach, we aim to marry the needs of each service user with support worker skills. We endeavour to maintain a consistent staff team and do not change staffing with the exception of cases of illness or if an emergency arises or temporary cover is required. We manage this type of situation with sensitivity because we recognise that continuity of care is paramount when delivering services. The service user has the option to change support staff that do not practice as determined by the care plan arrangements. We set individual objectives with each service user and carry out reviews with people who use our services to ensure that agreed outcomes are met. Care Plans are updated to reflect changing needs and objectives. We always ensure that Service Users receive clear written information about their Community Based Support and Enablement Services.
